Navigating Pacific Hearts (NPH) is a program that focused on the #1 killer of women, heart disease. In partnership with Pacific Islander Health Partnership (PIHP), ‘Ainahau O Kaleponi Hawaiian Civic Club (AOKHCC), Empowering Pacific Islander Communities (EPIC), Familia Pe Taha (FPT), Juon Wot Iakwe (JWT), Samoan American Youth of Orange County (SAYOC) and Kutturan Chamoru Foundation (KCF) funded by the University of California, Riverside, with support from the Foundation for the National Institutes of Health in partnership with the Heart Truth® program of the National Heart, Lung, and Blood Institute this project empowered Pacific Islander community organizations to assist women in identifying personal risk factors for heart disease. To accomplish this, NPH developed, implemented, and evaluated innovative programs that equip women to make sustainable lifestyle behavior changes that support heart health and lower their risk for heart disease. Through our programs and networks, we actively promoted awareness of The Heart Truth and the NHLBI’s national program for women about heart disease.
Goals and activities Navigating Pacific Hearts accomplished:
Creation of Heart Truth educational materials in-language and outreached to the NPH communities to increase knowledge of heart disease risk factors and risk reduction for Native Hawaiian and Pacific Islander (NHPI) women through focus groups and conducting educational presentations.
Increased NHPI women's ability to engage in heart health conversations with their physician and family members.
Educated and trained five NHPI groups by adapting the Kaleponi Pili 'Ohana Program (KPO) for Improving Lifestyle Intervention.
Hosted a special event for American Heart Month. The event was on Saturday, February 27th 2016.
